Unfortunately for us all, over time the skin begins to loosen on the face and neck. The jawline softens into jowls, and beneath the chin, another chin or vertical folds appear at the front of the neck. As our country’s aging population grows, it is understandable why facelift techniques have become the third most desired facial plastic surgical procedure. While the procedure was typically sought by women in their 50’s to 70’s, the trend in facelifts is to have surgery at a younger age. The new typical patient is likely to be in their 40s to 50s and may be just beginning to see some signs of aging. Facelift surgery has also become increasingly sought out by men, to retain a vital and youthful look that can help them keep a competitive edge.

In patient with minimal jowling and primarily loose neck skin, a necklift may be an option. I most commonly perform this procedure for my male patients. This has the advantage of shorter incisions primarily behind the ear and under the chin that are well camouflaged, and addresses the aging changes specifically in the neck. Not everyone is a candidate, as significant facial sagging may make a facelift the better choice, but we will discuss at your consultation whether this is a viable option for your rejuvenation goals.
Whether earlier or more advanced aging changes, a necklift can provide natural looking restoration of a more youthful neck contour. Depending on the extent of the surgery, the process can take a little over an hour. When the procedure is performed with a combination of mild sedatives and local anesthesia the patient will experience little discomfort. Some swelling and bruising are expected after a necklift, but much of it will diminish within the first few weeks.

In the past, lifting the face and neck could only be accomplished with incisions around the ear and removal of excess skin. Newer laser technologies allow not only for fat lipolysis but heating of the subcutaneous attachments and the dermis stimulating new collagen formation and progressive tightening of the skin. Heating of the skin with the newest 1440nm wavelength of the laser technology at Atlanta Face and Body Center, creates significantly more skin tightening than surgery alone or with prior laser technologies. This procedure can be performed via small punctures at the base of the earlobe and under the chin for access of the laser fiber. The result is fat reduction under the chin and of the jowls with tightening of the overlying skin with minimal downtime and recovery. It can easily be performed in less than an hour with local anesthesia and mild sedation.
The procedure is most suited primarily to two types of patients. Younger patients in their 20’s and 30’s who were candidates for traditional chin liposuction, but can now experience the added benefit of skin tightening to significantly enhance the neck contour results. The procedure can have a significant impact for younger patients with hereditary fat deposits that are resistant to diet and exercise. The other group of patients are those with early aging changes, typically in their 40’s and 50’s, who would benefit from the facial contouring effects combined with skin tightening to achieve the maximum improvement in face and neck contour possible without skin excision.
The effects of aging are inevitable, and, often, the brow and forehead area show the first signs. The skin begins to lose its elasticity frown lines, wrinkling across the forehead, and an increasing heaviness of the eyebrows develop. Even people in their thirties may have faces that look older than their years. Others may mistake your appearance as tired, angry, or sad when this is not how you actually feel. As a result, many people have opted for a procedure known as the forehead or browlift. Based on variations in how men and women age and incision placement, different methods can be used to perform this procedure. While all the various procedures will raise the brow and improve facial balance and harmony, some can also smooth the forehead and can even lower a high hairline. Every year, hundreds of thousands of men and women choose blepharoplasty to improve the way they look. Droopy eyelids can make you look older and can also impair vision. Blepharoplasty, or eyelid surgery, corrects these problems and also removes puffiness and bags under the eyes that make you look worn and tired. However, this procedure does not alter dark circles, fine lines and wrinkles around the eyes or sagging eyebrows. The good news is there are other alternatives that can address these concerns as well. Though blepharoplasty is often performed as a single procedure, I may also recommend a browlift, Enhance or EmpowerLift, or skin resurfacing to address other aging changes and achieve the best results.
In upper eyelid surgery, the incision is placed in the creases of the lids to minimize visibility as much as possible along these natural folds. In lower eyelid surgery, the incision is made in an inconspicuous site along the lashline and smile creases of the lower lid. The incision can also be made inside the lower eyelid if only excess puffiness due to fat pads needs to be addressed. Under normal conditions, blepharoplasty can take about an hour. Eyelid skin heals remarkably quickly and scars are usually inconspicuous with time. Bruising and swelling are expected initially but resolve over a few weeks resulting in your enhanced look.

Many women actually like the shape of their face better in their early 30s after losing a lot of their baby fat and starting to have more of a triangular heart shaped face. As one continues to age, we lose fat in the eyes, cheeks and jawline which along with weight gain in the lower face, neck, and jowls starts to invert the triangle so that the base of the triangle is now at the bottom of the face. Volume loss is most pronounced around the eyes, cheeks, and jawline, where fat grafting can create a more balanced rejuvenation of the face that may be combined with other techniques such as eyelid lift and/or a facelift. Conceptually, the goal of facial fat grafting is to invert the triangle so that the heart shaped face of youth returns. This can restore fullness to the “apples of our cheeks” and “lift” the cheek area without incisions or invasive surgery with risk of facial nerve injury as with endoscopic midfacelift or cheeklift. Volume restoration for the face also adds more light to the face, which in turn can make the skin look more radiant. As a minimally invasive procedure without traditional incisions, fat grafting has gained wide popularity. Fat grafting involves removing fat from one’s own abdomen or thighs to put into certain regions of the face that start to lose fat. Only tiny amounts of fat are removed so this should not be equated with liposuction.
